Cancer is actually your own body cells, its not a traditional disease which is caused by something from the outside (virusses, bacteria's, toxins, etc)
Every body has the potential cancer sells in them and they have this set timer which will then cause it to become trully cancerous at some point (as in, not function normally and divide to form a cancerous body, malign or not).
This timer is different in every person and thus it is very hard to say that "something causes cancer".. Because there are cases of people who never smoked getting cancer while people who do smoke never getting it.
The risk of cancer in such studies in this case only shows that the timer you have on your potential cancer cells have a chance of reducing, thus becoming cancerous early.

While there ARE viruses that can cause cancer, it's not the same as getting a virus and getting sick. (Keep reading)

Again, sort of wrong, sort of right. Cancer is something that's inherint in all cells, but it can be influenced from an outside sources.

Not really... At least not a timer in the sense of a countdown.
Cancer, stripped down, is the deregulation of function and unregulated growth (division) of cells (many cells with a set function don't divide quickly or even at all). ANY cell can become cancerous, but it can take time. Don't forget, there are actually several genetic diseases that cause cancer in children.
Basically, any mutation that either causes a growth gene to turn on or causes a stop-growth gene to turn off will make a cell inclined to become cancerous. (Side note: you have two copies of every gene. You only need to turn on one of the growth genes, but have to shut down both of the stop-growth genes, to cause an effect in that particular gene). It's generally hypothesized that you need to aquire mutations in multiple genes (baring the inherited ones previously mentioned) to have a cell truly turn cancerous.
The question then becomes how the genes pick up the mutations in the first place. Sometimes the DNA replication system is incorrect and can put in the wrong DNA base pair. Sometimes the editing systems that are suppost to catch this don't work right, and don't fix it. Like I've been hinting at above, outside factors can have an influence. Integrating viruses (not many do - HIV is one), if they integrate into a proper protoonco gene (a gene that can cause cancer when mutated, at which point it becomes an oncogene), can dirsupt the regulation and cause cancer. This is a big issue with our gene therapy vectors, because in medical trials it has caused cancer (before we understood that this was a serious risk...). Chemical agents (cigarette smoke, etc) can cause DNA bases to change their chemical "appearence" and cause mutations to be formed. UV light can also cause similar changes, which is why tanning is bad.
The chance a person is going to get cancer is based on a lot of things. Some of which is sheer dumb luck. Like Zuca said, some people can smoke forever and not get cancer. They're still accumulating DNA damage and mutations, just not in the genes that are necessary to start cancer growth. _________________ "People who are easily offended need to be offended more often."
